FAA Basic Medical Exam
To prepare for your FAA Basic Medical click the FAA Basic Med Link. There is a Basic Medical PDF at the bottom of the linked page. Print it and bring it with you. There is a portion for us to complete to finalize your exam.
DOT Medical Exams
To prepare for your DOT Exam click the DOT Med Exam Link. Print this, fill in the BLUE boxes, and bring it with you. The GREEN boxes are for us to complete to finalize your exam.
Coast Guard Exams
To prepare for your Coast Guard Exam click this Coast Guard Exam Link. Print the CG-719K, fill in section I and IV, and bring it the whole application with you. Please note that you have the most up to date version of the application, expiring in 2026.
