DOT Exams are paper based. Click this DOT Med Exam Link, print and complete the BLUE boxes. The GREEN boxes are for us to complete at the time of your exam.
Coast Guard Exams
Coast Guard Exams are paper based. Click this Coast Guard Exam Link to print the CG-719K, fill in section I and IV, and bring it the whole application with you. Please note that you have the most up to date version of the application, expiring in 2026.
